ID Multiple Field Item
1. Name on ID card (max 25 characters including spaces & punctuation) 2. Title on Card (located at bottom of front of card) – this is a drop down of either Educator or Student 3. School Name (max 35 characters including spaces & punctuation. If long, may be on two lines) 4. Grade Level – as in Challenge or 10th grade….Foundations or 2nd grade etc.. (max 25 characters including spaces & punctuation) 5. DOB – any combination is acceptable – examples include 2008, Oct 10, 10-10-2008 (pls note, for full birthdate use – versus /) (max 25 characters including spaces & punctuation) 6. Location – can be city or community or anything you want (max 25 characters including spaces & punctuation) 7. ID Number – this can be anything (max 25 characters including spaces & punctuation) (this is located on front and top of backside with strip) 8. QR Code – provide website address (this is located on back of card – code on example goes to CC Homepage) Please Note: this is optional but the spot will be blank if you choose not to do it. LOGO – your personal homeschool logo – JPG or PDF – JPG preferred (this is located on back of card) Name image with name of person card is for so I can attach the correct image. NOTE: If image is not squared, will be a gradient grey strip on sides of photo. 11. Verbiage at bottom of back. Note: this is optional but the spot will be blank if you choose not to do it. Example 1: This is the verbiage I created for an old card. “Curriculum and Community sponsored by Classical Conversations. Association with Classical Conversation is based solely on yearly membership. Homeschool Teachers are the primary educator using the Classical Conversation Curriculum.” Example 2: This is the verbiage on the above example. “(insert school name here) is a private school under Section 25.086(a)(1) of the Texas Education Code.” Please either provide your own verbiage below (max 125 characters including spaces or punctuation) or cut and paste whichever example suites you – feel free to change words up! Make sure you actually put in your homeschool if using example 2. 😉
